Solvit Application

This process is designed to make sure that we are the right school to serve the needs of your child and family. We recognize that the application requires a time investment on your end. Families who are thriving in this type of learning community often share that they truly enjoyed the process, and we hope that you do, too!

1. Download Learner Questionnaire

Please have your child(ren) complete the Learner Questionnaire. If your child is younger than 8 years old or isn’t able to write yet, a parent may serve as a scribe. For all learners 8 years of age and older, we want to hear from you!

2. Download Parent/Guardian Questionnaire

Please complete the Parent/Guardian Questionnaire.  All legal guardians must sign the form.  

3. Complete the Solvit Academy Application

Once you and your child have completed the Questionnaires, you are ready to complete the online application. You will upload them in the final two questions of this form. Once reviewed, we will respond within one week to set up your Family Meeting.
